WHO CAN APPLY FOR SERVICES THROUGH THE DDA?

A person who has a developmental disability can receive services from the DDA. The DDA provides services through its Medicaid home and community-based waiver program which:

 

  • Offers employment services, in-home support, and other comprehensive supports.

  • Provides lifetime support to people with developmental disabilities. 

DDA ELIGIBILITY APPLICATION FOR SERVICES OVERVIEW

  1. Submit your application with supporting documents (i.e., records, reports, other information that shows you have developmental disability), if needed, you can ask the DDA for help in completing your application. DDA Eligibility Application.

  1. Once the DDA receives your application, the DDA will contact you to schedule an interview.  You will need to select a Coordinator of Community Services (CCS), that will meet with you to conduct the interview. (Examples of a CCS include MMARSRC, Resource Connections, and Service Coordination)

  2. During the interview you will answer questions to help the CCS understand your needs and challenges. The CCS will also help you submit all the documentation to the DDA.

  3. The DDA reviews the information and recommendations made by the CCS and determines your eligibility for services and notifies you.

NOTE: The DDA considers young people ages 14 to 21 as Transitioning Youth (TY). The DDA suggests TY apply for DDA services while still in school this will help make sure there is not a gap in services when a student leaves school and needs adult services.

YOU’VE BEEN APPROVED, NOW WHAT?

1.  Next, you will fill out an application for the DDA Medicaid Waivers with the help of your CCS. (The DDA uses Medicaid funds to pay for your services.) The DDA has three Waiver programs for you to choose from:

2.   The DDA reviews your application and selects the Waiver Program best suited for you.

3.   The CCS then works with you to create a Person-Centered Plan for services and supports you need

and want based on the Waiver Program you have been accepted in.
4.   The DDA reviews your Plan. Once approved, you can obtain services.
5.   Your CCS will help you select the Provider that offers services based on your Plan.

6.   Throughout the process, your CCS will check in with you to make sure the services you are receiving meets your needs and wants.
